is it running off the top tensioner pulley or the bottom??
if its the top one something is out of alignment if its running off the bottom?? it sounds like the tensioner might be buggered putting the pulley out of alignment..

is it put on properly?

could be as troy said. or could be not on correctly thus not enough tension onto belt..
