Apart from milky white in the oil, what else to look for in a EF/EL engine with 200k km plus on the clock.

Check when the Head Gasket was done, if its been running with Coolant, with over 200kms it will tick, check if it blows smoke & service history..

Fords are tough, 200,000 is nothing just run in.....

now seriously... this really depends on the car, how was that 200+kays done? was it a country car or a city car. country cars drive longer usually at high speed so not as much wear on things then there is the city car. small trips and stop start traffic etc. so I guess you have to take each car on its own merit.
I have seen cars that look and drive like new with 350,000 on then and seen absolutre dogs with 250,000. so depends on how they are driven/looked after, serviced every X amount of kays yada yada.

by they way my car has 807,000 still on the original motor, has a few rough spots/war wounds from being a cab, but still keeps going
