Have u recently disconnected the battery. Usually takes a few weeks for the ecu to settle down and re-learn. I think clean/check/new ISC. My 94 ED 4.0L 5 speed does the same thing. Changed MAP sensor but nothing.

Sounds a little like my problem. I start my car sometimes and the revs will fling up and sometimes settle and drop so much that the car stops. Plus if you rev my car hard in neutral and then no accel., it dies too, lol.

Try cleaning you're throttle body

Yeah I did that just a few days ago, along with a whole lot of sensors. It's a little better, but still plays up at times.
