block needs resurfacing?
head isnt machined properly?
cooling system isnt doing the job?
are you using AU gaskets and bolts? Correct torque settings?

1. Get the head machined (and welded if it is looking bad around the coolant channels)
2. Check the block is flat
3. Make sure the surfaces are clean -and I mean CLEAN
4. use an AU head gasket
5. use new head bolts
6. assemble using a new or trusted torque wrench
7. replace or service the radiator

before you are doing all this check the condition of the spark plug tips. They can tell you if the engine is running lean or detonating which puts extra stress on the gasket.
