Combination of both.
I could cruise around at 4500 and use more petrol then at 2400. But if I loaded it up at 2400, i might use less petrol chaning down and loading up at 4500

The amount of fuel that goes into your engine depends on both rpm and load. In a world without mechanical loss a little more rpm and a little less load would just about cancel out and you would use the same amount of fuel. However, more rpm means the engine internals are moving more for every km the car travels, which is more friction that has to be cancelled out with power (more fuel and air). So lower diff gear = more rpm = more friction = more fuel consumption

Stuffed oxy sensor, vacuum leak, f**k injectors.
List goes on. Those are the first I'd check. Do a diagnostic on it, or pay someone else to do it for you.
