I was jsut driving along and i could hear a loud noise, was not exhaust, so i pulled over and the tire had come off the rim. well dead flat, what could of caused this to happen?

It could have just been a puncture on the wall of teh tyre or in the middle - then driving along for a bit on a flat tyre will definantly cause it to come off the rim
