ISC gummed up or dead, or idle control circuit inside ecu dead (STV-515 ecus had a high failure rate, STV-516 did not)

First I would try running a KOER test. If the revs come up when the test starts, the ecu is generally OK.

If they don't come up, then check to see if your ecu is STV-515 or STV-516. If it's an STV-515 I would borrow an STV-516 EF V8 ecu (eg 4DEG or 4DKF) and see what happens.

If you already have an STV-516 ecu then try another ISC
