wondering if anyone can give any suggestions on what is happening,

the wife's 95 nf failane 5.0lt about 270k's ,
transmission was clunking between 1st and 2nd gear change so i thought change oil and filter ,that problem fixed,
now even worse problem seems that when trans is put into reverse it sometimes slips into neutral same thing happens in drive , driving along then no noise just slips into neutral and free revs ,
the only thing that i have done to it lately was the above mentioned oil and filter change and the week before i changed the neutral switch on the side of the trans with a brand new one .

anyone have any idea what is happening and how to fix .
